An apparent cause analysis (ACA) is a process used to investigate the cause or causes of a safety event. This event might be blood stream infection, urinary tract infection, surgical site infection, Clostridium difficile, etc. 

ACA is designed as a quality improvement measure that is used to explain in evidence why an issue or near miss occurs and then to correct the issue and prevent its recurrence by creating an action plan that is assigned to confident individuals or groups within identified time period.
